Implementation and testing of new service to improve transition from paediatric to adult health care

A transition programme for adolescents with celiac disease is being developed by PP8 KUM in cooperation with the social pediatric center.

In Munich, a transition programme for adolescents with celiac disease is being developed by PP8 KUM in cooperation with the social pediatric center which closely collaborates Dr. von Hauner Children’s Hospital. The programme consists of three pillars: first of all, a special and individual visit with a pediatric gastroenterologist, a dietitian and a psychologist will be scheduled in the social pediatric center. During this visit, the adolescent will be able to address all questions and the specialists will assess the knowledge and self-capability of the adolescent to cope with celiac disease independently. Second, a workshop for adolescents will be held on 8th of February 2019. In this workshop, there will be interactive sessions to address, medical, nutritional and psychosocial issues will be as well as where to seek reliable information and support. A gluten-free menu will be offered to the participants. In parallel, a similar workshop will be offered to the parents of the adolescents to allow them to address their questions and sorrows.

Finally, a celiac passport is currently being designed to allow a complete and detailed documentation of the celiac diagnosis in order to avoid any unnecessary doubts and examinations in the future.
