Simulator training to improve endoscopist practice in field of CD

Diagnosing celiac disease in majority of cases relies strongly on histopathological examination of intestinal mucosa. To obtain adequate tissue samples upper gastrointestinal endoscopy (EGDS) with multiple biopsies must be performed. The procedure can be demanding especially in small children, where general anesthesia or deep sedation is required. Procedure is mostly performed by skilled pediatric gastroenterologists or in some cases by internal medicine specialists. However, patients are appointed for endoscopic procedure by many HCPs who can in their practice diagnose celiac disease. It is therefore crucial that both HCPs performing endoscopic procedures as well as those appointing patients for it have excellent knowledge about the principles of EGDS.

We are therefore aiming to improve skills of both groups of HCPs within the Focus IN CD project pilot.

For that purpose, we purchased a training model which closely resembles the anatomy of upper gastrointestinal tract, which can be used with the existing endoscopic equipment. With the use of this model participants of the training can learn about the anatomy of the gastrointestinal tract, principles of handling the endoscope, indications and contraindications for EGDS, appropriate tissue sampling and handling techniques and the importance of proper histopathological examination of obtained sample.

We designed a course for group participants, and have used a same model at two different occasions. One course was designed for students and trainees of medical schools attending the SkillsLab symposium in Maribor, and the second for students and practicing pediatric gastroenterologist, where the emphasis was on more advanced techniques. Participants were provided written material (lecture handouts, basic principles of endoscopy handouts, and Focus IN CD brochure for HCPs).

Both events were attended by 32 participants and all participants expressed high degree of satisfaction with our pilot activity.
